I think the title is more of a reference to the "ultimate" test of friendship/brotherhood: to give your dying brother/friend one of your kidneys. Most people reject this, and I feel the title "Kidney Thief" is sort of a way to say that the two people referenced to are in some partnership that is only beneficial on one side, and it's forced, leading the one who's kidney is stolen to feel cornered, and this is his cry for help. But, eh, I dunno.
