Velbloud is right, this performance is in the TV show Śaráda. It is probably its last concert in TV. The one in the church was later, it was its last public performance, but I never saw it. (Probably it was not filmed).

No, it isn't a concert in a church. I remember this performance which took place in entertaining TV show Šaráda (Charade) at the Czech Television studios. What's more, the instrument used here is a violoncello. So you mean probably a different concert.
I've read (on Wikipedia) that one of Petr Novak's last concerts took place in a church where he performed his old songs accompanied by a guitar and a viola. Is it this one? I think it might be but for me this place doesn't look like a church. I'm not sure, maybe he did more such concerts in different locations.
