It's on Stepmania. Thus, this is done with a keyboard. Not his feet. Trust me. I've seen worse. But in all possibility, this could be done with his feet also. It's just that the most likely scenario was a keyboard ;).

even with arrows on the keyboard, that's insane. that would take a crazy amount of practice, and personally?

This is step mania, not DDR. You play it with the arrow keys on your keyboard, not your feet.
