Well thanks but tried this too and a host of other tips I've found online and still nothing. Lost count of how many times I've replayed the entire level let alone the scene.. First game I am giving up on. Glad I only bought it for $8

when she was on her knees when you first see her, i kept saying to myself she's on our side, cause the firing thing was green, so then i thought everything was ok, then she throws them grenade things, and she's gone!

Thanks for posting! I Needed to see how long Michael would sit by the box and the length of the waves (as I couldn't immediately direct Michael to go Anywhere).
This video shows that Michael can move around on the box, and move from it after the first wave is killed. I stopped watching at 3:56 because I knew I could do everything else. Didn't know I was doing it on 'Realistic' this whole time though, got 'Rainbow Six Legend'. Always liked how quickly you die in this game on that setting.

i was playing this mission with a friend in realistic and at this part it happened a bug.
which was that: 1st she didnt apear then 1 min later she apeard downstairs and was in somekind of god mode
me and my friend died lots of times to have extra nades to throw.
that was the frst time we attemted this mission

to solve the you have failed to kill irena problem all you do is
1. send ur teammate to comp.
2. send other to box infront
3. you take cover behind computer and crouch.
4. toss smoke grenades at the box so it covers u and ur teammates.
5. use thermal and pick off enemies.
6. toss a few frags randomly on both sides.
7. when quiet, send teammate all over the place to make sure its clear of enemies.
(its common for an enemy to be hiding and not show up on map)
