Strabismic patient during alternating cover test: Demonstrates flick movement, which is pathognomonic for anomalous retinal correspondence (anomalous projection ). Patient is wearing Fresnel Prisms on glasses to estimate neutralization of the strabismus (eye turn). Note that non-dominant left eye shows the flick, and that after the flick, the right eye refixates from an *exo* posture. The amount of Base Out prism applied was likely over-correcting the esotropia (consistent with the exo OD). I believe the double fixation of the flick represents initial fixation with an anomalously corresponding point on the retina, nasal to the fovea (consistent with the first, temporal movement of the flick), followed by re-fixation with the fovea once the patient is viewing monocularly with the left eye (consistent with the second, nasal movement of the flick).
