The other option I see is tariffs, but those generally fall on the lower classes.

then that is the fault of the bill passer if something is found and exploited the government can go back and change, if they hide something for their own benefit I'm afraid it'll be found
and if it's something as extreme as fining for not having health care then I'm afraid they wouldn't be able to hide it that well seeing as bills are examined heavily on a daily basis by hundreds of different lawyers and legislators

why would they be fined for not having healthcare?
if they're gonna fine you for not having healthcare they may as well supply you with a healthcare by default. people will notice that problem and therefore the government won't fine for not having healthcare.

It wouldn't be the first type of insurance to be required.
As to taxing those with high incomes, it makes sense to place the greater part of taxes on those who can best afford to pay them.
