Yes B.Piper gave him the nickname. David mentions it on the Top Gear & so does B.Piper on the show with Justin (he even asks her how does SHE know xD).

I love how freely they joke on things like that. John always seems so at ease.

As an American, it really seems to me that other countries are a couple steps ahead of us in terms of tolerance. I don't think I'd ever see this banter happen on American T.V. - at least not without the straight guy acting artificially uncomfortable and making some joke or comment implying "I'm not homophobic, but I have to make it very clear that I'm not gay before I participate in this game."

That's because hardly any of our tv shows or movies that have a gay characters that isn't super effeminate as a lead. And when they do, parents get all homophobic and don't want their kids exposed to it. Not to mention all religious people who judge all the gay couples as sinners who shouldn't have the same rights as straight couples.
