In regards to the ESRB rant, things that give a game an AO rating are the sorts of things that you wouldn't see in games anyway. An hypothetical example would be a game that allows the extensive torture or rape other characters. The implied medevial torture of Amnesia or the raped underage characters of LA Noire or even the offscreen sex of Kratos and Aphrodite while naked breasts were onscreen is allowed in a mature game.

The one year between ages 17 & 18 is a pointless distinction, though.

For the record, the difference between M rated games and AO rated games is that most stores will NOT stock AO games. Though, generally AO games are basically porn titles, and most stores like Walmart and stuff won't stock normal porn anyway...I sort of lost my train of thought. The point is, graphic sex makes it AO, AO means stores won't stock it, stores not stocking it means no money, no money means no more games.
