Ok you say if is not red is dead. After you cut the black tissue you keep cutting red tissue, was that blood over dead tissue or it was real red healthy tissue?

@bambis46 The man is neuropathic,so the lack of feeling is not only commited to necrotic tissue ;) They are not cuticle scissors. Yes he removed phalangeal bone. Hope that helps.

The maggots will eat the dead tissue and be more accurate than the turkey carving the doc is doing on his foot, but not prevent the infection inside the foot. That is why his foot looks otherwise healthy. Diabetes is horrible, you see the guy can't feel it cause all the nerves in his foot are dead, not cause he's sedated.

@TrueVideo: Yes, maggot therapy can be very effective, and much more precise as they only eat dead tissue...unfortunately it is very difficult to get patients to consent to the treatment.
