There is patella alta (rare), mal-aligmnet of the tibial tubercle, congenital abnormalities, muscle imbalance and others. If the MPFL (medial patella femoral ligament) is torn than a reconstruction or repair may be needed.
How exactly are the ligaments tightened for medial plication? Do you know if some surgeons use screws or drill holes through the kneecap to tighten the ligaments in an open incision surgery?
There are a lot of options pending on what is wrong.
Sometimes the ligament needs reconstruction, repair or plication (shortening)
