Oh he has fear of death alright...in Renaissance Man, his program is nearly lost and gave his "death speech" to the crew on the holodeck.
and he briefly felt pain in Flesh & Blood
Maybe this was before he started to advance his programming to be more human?

@croquis24 yes, and during Robert Picardo's casting audition for Voyager he coincidentally said the "I'm a doctor, not a..." line without realizing it.
It's one of the reasons why he got casted as the EMH.

i think the doctor is the one person who changed the most over the whole star trek series from when he first appeared to his last apperance.
