I did not have the mag seated all the way. The mag release on this gun was accidently pushed in between rounds.

@eliasfrangoulis Actually, it turns out that the mag release was malfunctioning with any ammo. Currently Taurus is working on this gun at their factory in Florida.

@rayful23 +P means that the round has been loaded to pressures that exceed the original design specifications. For example, the standard .45 ACP round runs at a SAAMI max pressure of 21,000 PSI whereas a .45 ACP +P round is rated for 23,000 PSI. +P rounds should not be used in older guns, as they could have problems with hot loads. Almost any gun made in the last 20-25 years should be fine shooting +P rounds, but check with your gun manufacturer to be sure before you try them.
