Temp was 99.5 on all days.
IDK what the humidity was. I just followed the instructions. First 18 days filled one of the water wells 3/4 and on day 18 filled both all the way full. Didn't have an extra hydrometer.

so I open my incubator to candle them and it stank! day 14. I candled a egg that had a black mass on top and whenever i moved the black blob also moved. also this egg is cracked i think there was black goo or yelow at bottom of incubator. Do i take it out?

@FuntomArtist666 Yes, take out bad eggs. Bad eggs will build gasses and may crack or explode and they smell aweful.

This is probably where I have been going wrong I had washed 12 eggs and 2 hatched yet all had fully formed chicks in them. What do you think could this have caused this?

If you wash the eggs, germs can get inside the eggs easily. It could have been some kind of infection but it'm impossible to tell for sure. It could also be incubation conditions. Then, even when we do everything right, sometimes this still happens. If the membrane was tight around the chick (called shrink wrapped chicks) then the humidity was too low. If there was water in the air cell, humidity was too high. But there could be a number of reasons for them not hatching.

No, hens lay eggs with a natural protective barrier. When you wash the egg, you wash off the barrier. Never wash hatching eggs. Never buy hatching eggs from someone who says they wash their eggs. Sure, some may hatch but you will have best results from unwashed eggs. A little dirt or poo is OKAY but don't incubate filthy eggs, either. I let the incubator run for a day or so before hand to make sure temps are steady.
