I think george lucas ment to put that in the movie. In episode 2 If you watch jango fett go into his ship on Kamino while fighting obi-wan, he hits his head on the top of door

no, its an outtake. the original star wars movies were very low budget, and they didnt have the time or money to redo the scene

How the f$%k can it be an outtake if it wasn't taken out of the movie.
Yes, it was a mistake, but it was left in and was even given a nod (sic) in Attack of the Clones when Jango banged his head on the entrance ramp of Slave-1...
