What's wrong with Geoff?
The number "108" is merely Geoff's obsession, the number itself isn't particularly significant (in fact he could be obsessed with just about anything). The balloons are a physical manifestation of this obsession, I suppose in real terms they don't actually exist.
As the film develops the voice in his head gets louder and more confusing and the balloons become a greater burden. This represents Geoff's struggle with his obsession. At the end he becomes so frustrated that he casts away the balloons, this is Geoff breaking free of his obsession, you may have noticed that the voices stop at the same time.
Okay, so the final scene of the film shows Geoff becoming very distraught. He is free, but his obsession, maybe even his love has gone. Geoff is heartbroken, "108" was his love, then his obsession and finally it became the bane of his life. Now he has nothing, he is a husk of his former self, he is nothing!
