There's actually 2 more 4 character Bands not listed in any of the FAQs I've seen:

Rosa (Aim) + Rydia (Black Magic) + Izayoi (Illusions) + Harley (Throw Gil) for Temptation and (Risky Strike) + Luca (Analyze) + Edward (Salve) + Harley (Gil Throw) for Weapon In Progress.
