@tony8999 perhaps there were another vessels. Keel water shows that LPG alters to her port. While bulk carrier prefers to maintain her corse (?) Port to port usually. Or there had been an agreement made via radio VHF to pass starboard to starboard. Or a misunderstanding. Or another vessel made a bullshit. Or steering gear failure. And, finally, navigator's mistake. (sometimes they live the bridge though it's prohibited) Choose the cause you like))

It was a clear day! Both OOD's should be keel hauled. I have stood numerous watches as OOD. There was no excuse for these ships to collide.
