Crash the game with Ashei's Sketch and Ooccoo's Note. Items used so far to crash the game...

1. Bottle (doesn't matter what is in the bottle)
2. Ashei's sketch
3. Ooccoo's note.

Other items you can probably crash the game with...

1. Ilia's Charm
2. Sky Book
3. Wooden Statue
4. Auru's Memo

I think you can probably crash the game using any of those items.

Where can I find Ooccoo's note?

If you get ooccoo in a temple, then leave the temple without using ooccoo to warp out, you automatically get ooccoo's note. When you re-enter the temple, ooccoo's note goes away.

You can only get ooccoo in temples you have not completed yet. The temples you can get ooccoo are Forest, Goron Mines, Lakebed, Arbiter's Grounds, Snowpeak Ruins, City in the Sky. Ooccoo is not in Palace of Twilight or Hyrule Castle.
