About the ramekin part, do they need to be greased first?

No. Dry dry dry. You should serve it in the ramekin, so there's no need to grease it. And by Jorge there's enough fattening stuff on this already.
Guest should be allowed to lick the ramekins dry again if they wish (switch off the lights like I did).
