It's a cam with variable eccentricity along its axis (big blue part at 1:25), with two followers that drive the two big sprockets through ratchets. Depending on where you put the followers, they make longer (faster) or shorter (slower) strokes, changing the speed of the sprockets.
The two followers take turns driving the load to "eliminate" pulsations - while one is pushed by the cam, the other just rests on it (go to 2:33).
