I'll finish that sentence for you: you don't have any professional medical care nearby and your Thai guides aren't first aid certified. They probably don't have a spine board or even a first aid kit. The Krabi Hospital is a joke and the hospitals in Phuket are a long way off. This sort of climbing in cool... until something goes wrong, then you're completely fuct!
