i think it has something to do with actions speak louder than words because although the nurse does not say anything i think everyone was pre occupied with him... but hey who knows we could all be right or wrong its a David Lynch short
This whole film is actually a joke (in a good way) We struggle to pay attention to what the amputee is saying, but our attention is grabbed by the nurse. We try to listen to the amputee again, but we come in confused desperately trying to hear what she says again. Then the nurse steals attention again. The moment we can properly regain our attention, the film is over. We watch once more, and realize none of the dialogue even matches up. It means nothing.
It was shot twice on purpose as Lynch had to record the same thing on two separate videotape stocks that AFI was looking into buying one of, according to what he said in an intro to this.
