My experience is that mistakes are rewarded, but in a perverse way. That is, the guilty are rewarded and the innocent punished. That's the beauty of a modern bureaucracy: blame can be assigned administratively rather than factually!

From my point of view, assigning blame is (almost always) worse than useless. It makes the system as a whole worse. Everyone learns to thread very carefully, and to be very risk averse. This makes it very difficult to innovate and collaborate.
