yes, rider has good form and both made a decent approach, just not quite enough lift-off... No one's "fault"-- just a simple mistake (pulling a rail) gone wrong. When you pause the slow-mo, you can see that the horse stepped on the rail when it landed in front of him, which caused the pair to go...
