The piece is called The Saint's Hallelujah and is a mix of "When the Saints Go Marching In" and the "Hallelujah Chorus from Handel's Messiah". It was written for Canadian Brass. It has been recorded on many of their albums, including "Essentials" and "Amazing Brass" to name a few.
