There were three penalties on that play.
1) Hossa's dive from Neidermayer's bump,
2) Van Allen getting tripped,
3) Madden hooking the HELL out of Hossa.
Had Hossa not blatantly dove earlier, I'm willing to believe the ref wouldn't have turned a blind eye to Madden's hook.

@JimB667 I don't think its called separating the man from the puck considering the puck is already separated from him. When the second hit is delivered, the puck is at the ref's feet, who is a good 7 or 8 feet away from Phaneuf, who isn't even facing that direction.
