05/06/08 - Psych appointment/evaluation (423 lbs)
07/08/08 - 5 hour Orientation (398 lbs)
08/14/08 - Surgeon's Appointment (380.5 lbs) - H Pylori test
08/19/08 - CM Appointment - ended up just being a check in (383.7 lbs)
09/11/08 - EKG
09/16/08 - CM Appointment - I got a date! 10/15/08 (367.1 lbs) - lots of blood work
09/19/08 - Surgeon's Appt, sign consent (366.1 lbs)
10/9/08 - Pre-op TEACH and Nutrition classes, Physical, Pregnancy test (362.4 lbs)
10/15/08 - SURGERY DAY!!!1 (360.2 lbs)
10/24/08 - Phone appt with Nurse/Case Mng (missed initial call on 10/17, played phone tag) advanced to Stage 2 (I had already advanced myself.
