Had the same glitch shortly after the Karkand expansion. Happened once. Never again afterwards.

Amen to the crap matchmaking. Don't know how many times our squad get split up when joining, even if it's only two of us, server half empty or not. End up spending half the match just trying to switch team from the menu. Annoying as hell
