Just because a person with advanced dementia CAN swallow doesn't mean s/he WANTS to swallow... especially if skilled personnel are trained to put thickened puree at the back of his/her tongue so s/he swallows by REFLEX... especially if such "feeding" (not "eating") only prolongs the process of dying and increases the risk of more pain and suffering.
This reality prompts a rarely asked but most important question, a decision we should all make--given the risk of dying with dementia is 1 in 6 for those who reach the age of 55: "If you meet your own criteria for Advanced Dementia, do you want to continue ASSISTED SPOON FEEDING?"
This video is an edited version of a live presentation that Dr. Stanley A Terman gave at the San Francisco Bay Area End-of-Life Care Network on February 1, 2012. His initial plan was to spend more time brain storming ways to educate the general public that an easy way to complete Advance Care Planning, one that can effectively reduce suffering -- does exist, now that his colleagues and he can offer multiple strategies to overcome the many challenges to "Natural Dying."
Yet even this audience of professionals who work in the end-of-life field were not sure what choice they would make for themselves without more information.... So Dr. Terman changed the emphasis of his presentation mid-stream -- and now others can benefit from viewing the video.
The lively and interactive discussion, whose level is appropriate for the general public, can motivate individuals to do their own Advance Caring Planning and may inspire some to be even more proactive by spreading the word to family members, friends, others. Dr. Terman ends by listing some initial possibilities of people, from students to professionals, whom may be able to motivate others to complete Advance Caring Planning, in his concluding impassioned call to action.

Will clinicians who respond to 9-1-1 calls HONOR your wishes? Don't take a chance. Tell them exactly what you want them to do and NOT to do--in your words, while you still can.
This short video demonstrates how competent people can directly ask emergency medical personnel and emergency physicians to honor three physician's orders: 1) a Do Not Resuscitate (DNR) order; 2) an order NOT to start an IV (intravenous drip); and, 3) an order NOT to hospitalize unless needed for Comfort Care. These orders can prevent prolonged dying and an increased risk of greater pain and suffering as you die.
The video also briefly shows a "MyWCard" in a pouch that smart cell phones can use--to view, fax, download or print the physician's orders--as an alternative to going to the website, www.MyLastWishes.org and entering the patient's last name and date of birth.

To avoid days to weeks of unbearable pain and suffering as one dies from a terminal illness, and to prevent lingering for months to years in Advanced Dementia that can be associated with pain and suffering that may be unrecognized and therefore untreated or under-treated, Advance Care Planners can give their informed consent A) to Palliative Sedation to Unconsciousness and B) to Natural Dying.
This video is an excerpt from a SUMMARY INTERVIEW where the Advance Care Planner gave his informed consent--a strategic way to make sure that, in the future, others WILL honor his "Last Wishes."
