Hey man, just saw this vid the other night and kinda freaked me out. I tired to replicate what you were exibiting and I wasn't able to (luckily). But I also agree that you have a manufacturing problem with the head, plate or the screw for the lever needs to be tightened down.
I am glad it is now...
