Buy the CD, read the lyrics in the booklet. The song has no hidden meaning. It's not about heroin, regardless of what the title suggests....

Touring constantly and not getting sleep (they've always toured in a van, not a comfy bus) is so horrible for your voice especially if you don't know how to take care of it or if you've never learned proper technique to prevent damage...

@rutledge123 EP stands for "Extended Play", dumbass. It's a term used to describe a CD that contains more than just a single but isn't long enough to be considered a Full Length record. But, as it stands, it still is considered an album.
