I still have my original box, too. I don't know the date, but it was probably about 91 and I also got the free game deal. I chose Blazing Lazers from the list.
I have to brag a little that I still have the styrofoam inserts that came in the box though. lol

I have this game for my TG-16. I've beaten it a couple of times, but I agree with needing to hold on to your powerups. The only way I've been able to beat it is by using only one life. I got to where if I died at any time, I would just reset and start over.
