I find the music to voice ratio obnoxious. The music shouldn't be so loud. The end bit sounds much better than the rest of the song (guitar and voice only).

Looks like it's only that one ear, there must be something wrong with his middle ear, it is meant to have a fluid in the auditory tube.

The auditory tube links the ear to the back of the throat, so without the fluid, (or little fluid, I guess) it's possible for smoke to pass through.
