You fill the form, submit it, and within a week or so YouTube decides whither you fit for a partner program or not. It depends on you channel's number of views. Don't know exactly what is the criteria, but your total views should be thousands, something like that.
You have to become a partner with YouTube, then you get few new branding options available, like a banner on top and a side column at bottom right. These options are not available for a regular account.
