Looks to me like the aircraft was possibly overloaded (weight) for altitude, also, maybe had the pilot attempted to transition into forward flight early rather than climb vertically he may have had more success.

I think it's both; side-slip, then retreating blade stall at the end. Don't forget the effects of precession would cause a pitch up, not roll to retreating blade side.
