The New Hampshire Citizens Health Initiative, in partnership with groups from all parts of the New Hampshire healthcare system, launched the Multi-Stakeholder Medical Home Pilot in 2008. As part of this pilot, nine sites throughout the state are participating in a 2-3 year study of the patient-centered medical home. Pilot sites include primary care practices with nearly 100 clinicians, 39,000 commercially insured members and more than 130,000 unique patient visits per year. The medical home concept re-centers health care on the patient's needs by pro¬viding primary, preventive, and chronic condition care that is personalized for each patient. It emphasizes care coordination and health information technology, including electronic health records, to help prevent and manage chronic disease. The medical home strengthens the patient-physician relationship by allow¬ing the doctor and team of health professionals to spend more time with each patient to develop and follow through on an individualized plan of care.
