As much as you may not want to hear it the rotors are probably warped. Loose suspension parts can exaggerate the problem but they are not the cause. It's a little known fact but many service manuals say to machine new rotors before you put them on the car but the ideal situation is to machine them on the car with an on-the-car brake lathe.
