You really seem to have left out some serious contenders. The Iranian Air Force, The USN and the USMC Air Wings (both should by rights be in the top 20), South Africa, Argentina and Chile also merit some consideration.

Sighs, The A4 was the chase plane ,with cameras, to film the drop of ordnance from a new rack, The rack and bomb separated from the F/A-18 and flew in a corkscrew shaped path shearing off the right wing of the A-4. If the bomb had dropped normally it would have gone done, not sideways.
