Spoiler Alert: Captain MacMillan was a soldier in the 22nd SAS Regiment in Call of Duty 4 Modern Warfare. He is seen during the mission "All Ghillied Up" and "One Shot One Kill". In the mission him and Captain Price were ordered to assassinate Imran Zakhaev. After the assassination attempt they failed not knowing and were forced to leave Pripyat (a ghost town in Ukraine due to the Chernobyl accident). While escaping Captain MacMillan got downed by a Mil Mi-28 Havoc.

Shangri La is the mission Crash Site (I think), because they have a crashed plane in that mission aswell as the new zombie map itself. Like you said they took the element 115 from Call Of The Dead and got shot down at Shangri La. Where did they find a plane from in Call Of The Dead though? They might have took it from another place. Other than that when you save the original surviviors (apart from the U.S Marines), they went to Shangri La because the element 115 was there. This will be amazing!
