Pilots Forget to Turn On Seatbelt Sign!
The seat belt sign was off the whole time during taxi, until the pilots figured it out about five minutes before takeoff. It was an A320 by the way.

@jspivak86 in case something happens during refueling. Wheelchair passengers also can't board the a/c while refueling as it'll be extremely hard to get them off the aircraft if something did happen.
