The third season, with "celebrity" captains, "commissioner" Rip Torn and worst of all, hour-long matches, completely jumped the shark. It was painful to watch, especially the playoffs.

That being said, I think what did them in more than anything else was the neck injury to the L.A. girl. I'm sure that freaked out GSN's lawyers to the point of cancellation (much like the 90-degree broken ankle in Slamball on Spike TV).
