All these people assembled for the purpose of hearing Hitchens speak, so Hitchens is standing in the front and has as a microphone. Unlike Republican Rallies the heckler got a lot of attention and patience indeed. The heckler fully understood Hitchen's repeated proposition to remove him personally and consciously provoked the consequences.
It is simply not practical for people to speak at the same time and at times when anyone in the room -1 person is there not to hear the audience member who challenges the speaker. Attention can only be granted to one person at once and disturbs anyone else and the heckler wants to take attention away from the speaker otherwise he would choose any other time, any other place to convey his message.
