left wrist bowed at top and club face still open
swing plane a little steep.
It looks like you have pulled the club head inside too fast.
hands, club head and ball must be on same plane.
BANDAID
make grip stronger, flatter swing
pull the club head back straight back the first 12 inches before going inside.
