I know the one with the sheet is when Sam is kidnapped from Alexis's house by Manny and held in the sub-ground floor of the hospital, Jason is shot in the shouder and saves Sam and than kills manny by throwing him off the roof. Jason dreams that Sam was murdered by manny, that is were the sheet comes up.
I'm trying to remember the date, i'll get back to you if you still have not seen it. I watched that episode like three days ago, i know it happened in 2006 fall.
