When teaching students mechanic courses the first thing you learn is SAFETY!! At least when I did my courses that's the way it's done. The exhaust should always be connected to a duct pipe that runs from the exhaust pipe to the outdoors even if you will be running the engine for a short period.
I had once experience like this. I was on garage, starting my new built engine, and running camshafts in. Then I saw intake Camshaft bolt opening quite fast! Quickly as I could I turned ignition off and that second, what the engine took to stop, felt like a minute! The bolt did'nt come off and the wheel stayed on the pin. It would have been severely fucked up...

@marek0086 Because there can be extra clearance in the internal rotating parts When there is NO load on the engine while red lining it, our engines are piston (reciprocating piston) engines. That means: that when a piston goes up it's inertia takes up the minute clearances in the wrist pin, crank to con rod, etc. in one direction. Then the piston goes down and the reverse happens. Under load, this effect is very much minimized because piston assembly inertia(etc.) is damped by the load.

whoa dog... if you sneak up on the rev limiter no damage will happen assuming you don't overheat the engine... it's when you snap from idle (650rpm) to redline (8000rpm) with no load that equals spun main bearings!
