i made one of these out of PVC but i cant seem to be able to get it to stay lit at all places at once. Any suggestions?

I think i may have a few answers
1) is the pipe small enough? there might be too little air pressure inside the tube.
2) normally, if one part of the tube blows out the other flames will re-light it, so your holes may be too far apart.
3) is there enough gas getting to all the holes?
4) if you are playing very low notes or very high notes, some parts of the pipe may reamain unlit
Hope that helps!

Awesome effects. One side note, your Wikipedia link points to an old version of the page. Not sure if its editable at this point, but figured its worth mentioning.

I belive the quality has to do with the amount of propane compaired to the volume of the holes. I am no expert, however, it's just an educated guess.
