  • holy shit.

  • very good video.thanx doctor v. reddy.

    dr rakesh shukla

    gorakhpur

  • Mitral Vavle ping pong...:)

  • a big one

  • scary!

  • wow she need open heart surgery STAT

  • I'm a student currently on my externship. And I gotta tell you... (not at the expense of the patient but) that's sooo.... cool? I mean is this normal that I find this cool. Now that's a bloodclot.

  • omg thats crazy. I wonder if she is still alive

  • clot that big could take out a carotid.... yumm...

  • @Lachy101 a carotid? I doubt it'd make it that far. its almost twice the size of the aorta, and the MV is stenotic. Its almost safer at that size :) At least, acutely.

  • Question raised is... when performing surgical intervention for MS, is there a high risk of an embolic CVA? We are not able so any other small thrombi with 2D. Great video, thanks for posting.

  • Yeah, what can be the appropriate treatment for cases like this?

I was thinking that if she takes thrombolytics (e.g. streptokinase), the clots SLOWLY dissolves right?

Then when it gets small enough to pass through the mitral valve, it could be possible that it would reach the lungs..

  • Thrombolytics would be entirley innapropriate (not to mention ineffective!)

She has a rheumatically stenosed mitral valve with organised thrombus formation, despite the presence of sinus rhythm.

The management is open mitral valve replacement. Anything else is idiocy.

  • it will reach the lungs? From left side of the heart?lol....

  • How would it reach the lung! Throught the mitral valve??? It is in the LA!

  • @ricwilliam lol about the lungs.

    how about brain attack?

  • pinball!

  • what are the risk of having that?...what is the treatment required for that condition

  • i would say that it is a clot, formed long time ago (you could see dense material close to the center of a clot).Clot formation was caused by turbulent blod flow.Main complication-RHD,secondary-MVS.treatment plan-surgery for MVS with clot removal.

  • This looks amazing indeed!I wish there would be a four chamber view as well...;-)

  • watching this, strauss' "the blue danube" comes to mind :-)

  • It looked like a pinball machine.

  • Did she have any risk factors for a thrombosus. SHE'S in A-FIB!

  • Damn, I am drowning just watching this video. So the management is surgical huh? No blood thinners for this patient? Thanks for posting this. Very Interesting.

  • Blood thinners in this case would be a joke.

  • Hi all

RHD is most common in 2nd or 3rd decade

youngest patient reported in three year age.

In fact there are 100s of articles on

    juvenile RHD

    dr venkat

  • 26 yrs...really rare....impressive find

  • Thank you very much for your video...

Go watch my video and give me your comment. Thank you very much and good day

  • Filed under: Not seen everyday :)

  • It seems like the mitral valve is prolapsing.. and playing ping pong at the same time... thank you for sharing

  • It looks more like a myxoma that dislodged or broke loose rather than a clot.

  • not really.. it has a fairly homogenous appearance and clot is consistent with the evident mitral stenosis.

  • Gee, ya think it hurts?

  • impresionante , espectacular

  • That is incredible!

  • Awesome!

  • WOW!

  • Extremely good demonstration of Mitral Stenosis with associated thrombus. Very unique.

  • Yes and strange that she has MS at 26

  • Dunno about the human side of things but the IVS/PW sure do look thin. ?DCM?

Very nice mass/clot. In dogs oral anti-coags are used as well. Lucky the girl had mitral stenosis otherwise she may well have died before the clot had a chance to dissolve!

    nice one!

  • Myxoma would be attached to IAS. What did the biopsy show?

Great case!! Hope the girl did all right

Sadly, as one of our docs says "The more "Wow's you hear from echo techs, the worse the patient's prognosis is"

  • Great stuff!

  • That looks more like a left atrial myxoma and she had rheumatic fever with mitral stenosis that doesn't appear that severe based on the mitral valve morphologhy.

  • What was it in the end- on surgery?

  • Unbelievable! Thanks for posting!

  • That's pinball.

Fasinating but I hope everything goes okay!
