This scene in this movie, was brilliant when it comes to horror. Notice in the background, as the turns is fiddling with the door, the two folks in the background leave. The cop comes in, and goes off camera, and as soon as the nurse turns around, and takes a step, its the loud music, with the (Demon) doctor in sheets with what seems to be hedge clippers following her. I also find that there is a psychological anxiety inherent to the scene's composition in that everything is grey and static except the nurse moving around. her red coat...

Much as you come to know that when there's an obvious build-up of tension in a horror scene with swelling music and a dumb girl walking slowly to investigate a noise, it will likely turn out to be nothing (or something inconsequential).

At first it is static and inert, but on further inspection, the image becomes animated through the sheer economy of gesture, much like an ant carrying a piece of food on a flagstone pathway seems to give animation to the whole quiet of the landscape...
