The song and the video are about the horror and viciousness of sex. The video is a reverse-gender rape. The girl cuts him a gash to use as a vagina and impregnates him with "swimmers/seawomen" [as opposed to seamen]. It's a reversal of roles, and commentary on the the fact that sex IS an act of violence in itself. Even if it's wanted and welcome, it's an invasion.
