He was overweight, he was having some lung and breathing problems (like coughing up blood clots), he was drinking heavily, he was using cocaine. Any of those things could have lead to his death... and he might have accidentally done heroin, even in a small dose, with all the drink in his system, it could have killed him.

Pam gave him heroin and even admitted it to the police when she called them. I am not disputing that he was in bad shape but that only further emphasizes my point that his death was anything but "mysterious." Anything you named could have "lead" to his death but the likeliness is that all of them+accidental intake of heroin=death.
